Nature of the Services Provided

We Provide Care

Sawbridge Primecare exists to bring helpful and friendly care services to Clients living in their own homes. Our registration with the Care Quality Commission enables us to provide care in the following regulated activity, as defined in the Health and Social Care Act 2008 and Care Act 2014.

Sawbridge Primecare Ltd provides services for the following bands of Service User:

Age Groups:

Adults aged 18 – 65

Adults aged 65+

Basildon, Braintree, Castle Point, Chelmsford, Colchester, Epping Forest, Harlow, Maldon, Rochford, Tendring, Uttlesford

Care Plans and Risk Assessment

We feel that successful Support Planning impacts significantly on the welfare of the Service User. We have a Support Planning System that is Person Centred and fully detailed and designed to meet all aspects of care for the Service User. The layout of these is user friendly and meets all aspects of Dignity, Choice, Respect and Privacy of the Service User.

At every stage of the Support Planning Process we welcome input from the Service User, family, friends and Care Management Professionals.

We commit to review Support Plans and Risk Assessments on a monthly basis or as required and make any necessary changes that are agreed with the Service User. There may be occasions when we need to make changes and where the Service User is unable to communicate with us, we will seek advice and commitment from family and representatives. It is important that all parties involved in the Service User’s life are involved in this review process.

Our ultimate objective is to provide well established, well prepared and informed Support Planning for our Service User to empower them in a safe and agreed way.

Therapeutic Activities

We take pride in supporting our Service Users’ normal social network and social activities. The Service User’s Care Plan includes a facility for recording life history, social networks and contacts, and preferences for activities and hobbies of their choice where they can be offered access to those networks and activities whenever they wish to do so.

Personal Care

Personal care is support that involves physical contact between carers and service users – this can include Oral hygiene, washing, shaving, dressing, grooming, toileting and continence care, all within the comfortable and familiar surroundings of your own home. Our staff team are all trained in the latest safety techniques to physically assist throughout personal care and will help you to maintain their modesty at a level they feel comfortable with.

Care For Older People

Whether you or a family member needs a little extra help around the house, assistance at either end of the day, or 24-hour support to live at home, Sawbridge Primecare Ltd can help. Care for yourself or an elderly relative can be provided in the comfort of your own home. Our homecare services can support seniors with those every day tasks that may have become difficult to manage. Daily activities like housework, shopping, cooking meals, dressing, bathing and personal and continence care may not be as easy to complete as they once were but our specially trained and friendly staff team can help.

Dementia Care

Our friendly and trusted team staff can ensure the home environment remains safe and advise on any adaptions that will make life easier. We also work with you and your loved ones to provide emotional support and respite, to access local services and to adapt your care package to ensure changing needs are anticipated and always met.

All our staff team have been trained by specialist Nurse Trainer to deliver free information and awareness sessions to individuals across the local community, helping more people understand what it is like living with dementia.

We understand, we care and we can help by supporting you to remain living independently in the comfort of your own home.

Specialist care

Specialist care is providing care to those with special conditions that require additional expertise or specialist equipment. Specialist care covers neuromuscular disorders, spinal injuries, cancer care and more.
We believe that spending time with loved ones is important, which is why we provide specialist care from respite or regular visits, through to full-time homecare.

Living Care

Live-in care is when we offer a 24 hour care and support service at home. This allows your loved one, to stay at home in familiar and relaxed surroundings with a unique level of one to one support that includes personal care, housekeeping and companionship, provided by a team of experienced staff.

Respite Service

Our range of flexible respite service offers you some choices of short breaks of either a morning or afternoon break whether it is a one off or once, twice a week where it leaves you free to spend quality time with your friends and families or to attend to your immediate needs.

Companionship

Our companionship service offers an individualised support package that can reduce the risk of you becoming socially isolated. Depending on the type of assistance needed we can provide a range of companionship service such as:

• Social interaction in the community
• Support with using public transport and escorting on journeys
• Respite for family carers
• Company whilst doing hobbies
• Food preparation
• Light housekeeping
• Light gardening
• Emotional support
• Help with everyday tasks, shopping for food or clothes
• A friend to listen and talk to
• Support on a day trip or even further afield

Home Services

We understand how maintaining your routines is important to you in order to continue to lead an independent and dignified lifestyle. We offer a variety of safe home services that will assist you in simple tasks that you are now finding difficult to do on your own, things such as changing and making you bed, putting them to wash including your other clothing items, painting and decorating, gardening and many more.

Domiciliary Care

As a domiciliary care service provider we are regulated by the Care Quality Commission (CQC) and we can support our clients with personal care and support services described below.

Long-term physical or medical conditions where you may require constant care and support whether you live at home, in supported living accommodations, residential care homes and day care.
